I used a 480exII with a homemade diffuser, mounted onto a bracket so that it could rest directly on top of the lens. I used a second fluorescent light resting on the table next to the whole setup, so I could see what I was looking at for focusing. The bubbles were made using a piece of tupperwear that I cut apart. Black shirt for a backdrop.
